Who we are

Tiffany M Holm FNP Registered Nursing A Professional Corporation, trading as OmOmbre, is an online mental health practice based in San Diego, California. Care is provided by Tiffany Holm, Family Nurse Practitioner.

What this policy covers, and what it doesn't

This policy describes how this website handles information about people who visit it.

It is separate from the Notice of Privacy Practices, which governs your health information once you become a patient and which is provided to you during intake. If you are already a patient, that notice — not this page — describes your rights over your medical record.

This website does not collect health information

This website is informational. It has no patient login, no forms, and no facility to submit medical information. We do not request, receive or store protected health information through this site.

Please do not include medical information in an email to us. Ordinary email is not a secure channel — it can be read in transit, and it sits on servers we do not control. Once you are a patient we will take your history through a secure process designed for it.

What we do receive

If you telephone or email us, we receive whatever you choose to tell us — typically your name and contact details, and your reason for getting in touch.

We use that information only to respond to you and, if you decide to become a patient, to begin your care. We do not sell it, rent it, or share it with anyone for marketing, and we do not use it to build a profile of you.

Enquiry records are kept only as long as needed to respond and to meet our record-keeping obligations. If you become a patient, your information moves into your medical record and is then governed by the Notice of Privacy Practices.

Cookies, tracking and analytics

This website does not use advertising pixels, marketing trackers, or third-party analytics that identify you.

This is a deliberate choice. On a mental health practice's website, the mere fact that a particular person visited a particular page can itself be sensitive, and we would rather not create that record at all.

Your browser may still store ordinary technical information needed to display the page. Our hosting provider keeps standard server logs, which may include IP addresses, for security and reliability.

Links to other services

Some pages link to services we use, such as our payment provider. Once you follow such a link, that company's own privacy policy applies to what happens there. We would encourage you to read it.

Children

OmOmbre treats children and adolescents, but this website is not directed at children and we do not knowingly collect information from a child through it. A parent or guardian should make contact on a minor's behalf.

Your choices

You may ask what contact information we hold about you, ask us to correct it, or ask us to delete it. Email hr@omombre.com or call 619-323-5146.

Requests about your medical record are handled under the Notice of Privacy Practices rather than this policy, and we may need to verify your identity before acting on one.

California residents have additional rights over personal information under California law. Medical information held by a health care provider is separately protected under the Confidentiality of Medical Information Act.

Security

This website is served over an encrypted connection and kept up to date. No website can promise perfect security, and we will not pretend otherwise — which is another reason this one is built to hold as little information as possible.

Changes to this policy

If this policy changes we will update this page and change the date shown at the top. Material changes will be reflected here before they take effect.
